Forms to Request Service on SAG National and Local Committees
Interested in serving
your Union? You must meet these eligibility requirements:
- All National and Local Committee members must be active, paid-up members of
Screen Actors Guild to serve and participate in meetings.

- Dues delinquent members will not be eligible to serve or participate in meetings until dues have been fully paid.
- Local Committees require that you be a member of the Division or Branch (e.g. Hollywood, New

If you are eligible and would like to serve on a committee, please complete the appropriate Committee Service
Request Form for your Division and return it to the Committee Office via email,
fax, mail or in person.
