Filing a Claim Inquiry

CLAIM INQUIRIES
A claim inquiry is an allegation that the signatory employer on a SAG production has violated the collective bargaining agreement in any way. Once a claim inquiry has been filed with SAG, it is investigated by members of the Contracts Team to determine if it is valid with respect to the apparent facts and relevant contract provisions. If the claim inquiry has been determined to have sufficient validity, it is filed by SAG with the employer as a formal claim. If it has not been determined to be valid, the inquiry is closed and the performer(s) who submitted the inquiry is advised of the Guild’s decision and the reasons for the determination.

Please fill out the form completely, in order to prevent any delay in processing your claim inquiry. Be sure to indicate the claim issue involved under "Description of Claim", as well as any further information that may assist us.

  1. LATE FEES: Does the lateness involve a production salary/session fee or residuals? If your production salary was paid late, include a copy of your check stub(s). If your residuals were paid late, please include copies of all applicable check stubs.
  2. PROBLEM WITH RESIDUAL PAYMENTS: Include copies of all residuals check stubs, including any residuals previously paid. Provide all known cycle dates if a commercial.
  3. PROBLEM WITH PRODUCTION SALARY/SESSION FEE PAYMENT: Include a copy of your contract, as well as any check stubs you have already received.
  4. UNAUTHORIZED USE OF COMMERCIALS: Did you see your commercial air but haven't received payment? It is necessary to provide us with date, time, station and city where the commercial was sighted, to the best of your knowledge.
  5. IMPROPER DOWNGRADE/OUTGRADE IN A COMMERCIAL: Attach your headshot. Please describe what you were wearing on the shoot day, including wardrobe as well as any hair goods and prosthetics. If you have a copy of the commercial, include it as well.

Once we have received your completed claim inquiry form it will be assigned to a Business Representative. The Representative will review the claim inquiry and process it accordingly.

You must also attach a copy of your production salary/session fee pay stub (claim inquiries received prior to production salary/session fee being paid will not be processed). Attach a copy of your contract/voucher. Please describe what you were wearing on the shoot day, including wardrobe as well as any hair goods and prosthetics.
  2. LATE FEES: If you are filing a claim inquiry for late payment of production salary/session fee, you must attach a copy of your check stub and (if you have it) a copy of the envelope the check came in (with the postmarked date visible).
  3. ALL OTHER PRODUCTION/SESSION ISSUES AND PAYMENT ISSUES: Include a copy of your contract/voucher.

Once we have received your completed claim inquiry form, it will be assigned to a Business Representative. The Representative will review the claim inquiry and process it accordingly.
