MEMBER ALERT: DATE OF BIRTH NOT REQUIRED ON PAYROLL VOUCHER FORMS
(April 23, 2008) Screen Actors Guild has been made aware that on several of the payroll companies’ new voucher forms there is a section that asks for members’ birth date, with some forms indicating that the payee’s date of birth must be provided in order to be paid.
